Trans Abstract

Purpose

To evaluate effectiveness of ultrasonographic measurement of carpal tunnel by anatomical area and correlation with electrodiagnostic study in diagnosis of carpal tunnel syndrome.

Methods

From September 2018 to March 2019, we performed the ultrasonography for 30 cases with carpal tunnel syndrome diagnosed with electrodiagnosis and 30 cases as control group. We measured median nerve diameter, cross-sectional area (CSA), and flattening ratio (FR) by area of carpal tunnel. We analyzed the difference of measurement between two groups and correlate the measurement with electrodiagnosis findings.

Results

There was significant statistically differences in sonographic measurement between two groups by independent t-test (CSA zone 1, p=0.01; FR zone 2, p=0.000; FR zone 3; p=0.001). With Pearson correlation test, there was correlation between sonographic measurement and electrodiagnostic findings (terminal latency and nerve conduction velocity) statistically, but the Pearson coefficient was low (r<0.4).

Conclusion

By anatomical area, the available value of sonographic measurement was different. But, as the values were has low power to diagnose the carpal tunnel syndrome, ultrasonography is proper to use as a complementary tool in diagnosis of carpal tunnel syndrome.

서론

수근관 증후군(carpal tunnel syndrome)은 상지에서 가장 흔한 포착성 신경병증으로서 수지 및 수부의 감각 및 운동에 영향을 미쳐 증상을 일으키는 질환이다[1]. 전통적으로 엄지부터 환지까지 이환하는 통증과 저린감, 밤에 악화되는 통증, 무지구 위축 등의 증상을 확인하여 진단을 시행하였고 더불어 전기진단검사(electrodiagnostic study)를 확인하여 진단의 확진 및 감별을 시행하였다[2]. 최근 들어 근골격계 영역에서 초음파적 진단 영역이 점차 확대되어 가고 있는 가운데 초음파를 이용하여 수근관 증후군을 진단하기 위한 많은 연구들이 있었다[3-8]. 이러한 연구들 중 많은 문헌에서 진단기법으로서 제시되어 왔던 것은 횡수근인대(transverse carpal ligament)로 정중신경이 진입하기 전 혹은 진입 이후에 비대해진 단면적을 측정하는 방법과 횡수근인대에 의해 정중신경이 압박되어 있는 비율(flattening ratio, FR)을 계산한 것으로, 많은 문헌에서 이러한 측정값의 진단적 의미를 강조하고 있으나 측정값의 참고치 및 이에 대한 민감도와 특이도는 문헌마다 상이하고 초음파 측정의 기준 위치도 수근관의 근위부인지 원위부인지에 대한 합의점이 없고 모호한 경우가 많다[9]. 또한 수근관 증후군 진단 시 시행되었던 전기진단검사의 중증도와 초음파 소견의 상관관계를 분석하려는 연구도 진행되었으나 이 또한 합의점이 없다 하겠다[6,8]. 따라서 저자들은 해부학적 측정 위치에 따른 각 측정치의 유효성 및 전기신경검사와의 연관성을 확인하기 위하여 본 연구를 진행하였다.

대상 및 방법

1. 연구대상

본 연구는 보건복지부 지정 공용기관생명윤리위원회의 승인을 받았으며(No. 2019-2398-002), 심의 결과 동의서는 면제받았다. 2018년 9월부터 2019년 3월까지 수근관 증후군의 증상으로 내원하여 초음파적 진단 및 전기진단검사를 시행한 30증례(A군)와 다른 질환(내상과염, 외상과염, 상지의 연부조직 종양 등)을 주소로 내원한 환자의 처치 시 수근관에 대한 초음파 계측을 시행한 30증례(B군)을 대상으로 본 연구를 진행하였다. 연구대상에 대한 통계학적 자료(demographic data)는 Table 1에 정리하였다.

2. 계측의 방법

초음파적 계측 시 8-17 MHz, 초고해상도 선형탐촉자(extreme high density linear probe; Alpinion, Seoul, Korea)를 이용하였으며 정형외과의사인 한 명의 측정자에 의해 측정되었다. 수근관의 경로에 따라 횡수근인대가 시작되기 전 근위부를 1구역, 주상골과 두상골 사이를 지나는 구역(proximal carpal tunnel)을 2구역, 대다각골 및 유구골 사이를 지나는 구역(distal carpal tunnel)을 3구역으로 구분하여 각각의 영역에서 계측을 시행하였다[9] (Fig. 1).

3. 계측 지표

계측 지표로서 정중신경의 직경(가로 D1, 세로 D2), 둘레(C), 단면적(cross-sectional area, CSA)를 측정하였고 가로 직경 및 세로 직경의 비로서 정중신경이 압박되어져 있는 비율(FR)을 계산하였으며 수근관 내부 정중신경의 단면적과 수근관 이전의 단면적의 비(CSA ratio: zone I/zone II, zone I/zone III)도 계산하였다[3] (Fig. 2).

4. 전기신경검사

전기신경검사는 한 명의 신경과의사에 의해 시행되어 측정된 말단잠복기(terminal latency, TL), 신경전도속도(nerve conduction velocity, NCV; wrist to palm, NCV(P) and wrist to finger, NCV(F))를 이용하였다. 이는 전통적인 수근관 증후군의 진단기준인 (1) 손가락-손목분절의 비정상 감각신경전도, (2) 손바닥-손목분절의 비정상 감각신경전도, (3) 말단잠복기 지연을 따른 것이다[10]. 전기신경검사의 중증도 및 초음파 계측치의 연관성을 분석하기 위하여 신경전도속도에 대한 선형관계 분석 및 중증도 분류법에 의한 등급에 따른 초음파 계측치의 차이 유무에 대한 검정 역시 시행하였다[6-8]. 중등도 분류법은 Steven [11]에 의한 분류법과 Bland Scale [12]을 적용하였다(Tables 2, 3).

5. 통계학적 분석

A군과 B군 표본에 대한 동질성을 평가하기 위하여 나이에 대한 independent t-test를 시행하였고 좌우 및 여성과 남성, 양손 비율에 대한 검정을 위하여 Fisher exact test를 시행하였다. 양군의 계측치에 대한 평균 비교검정을 위하여 independent t-test를 이용하였다. 초음파적 계측치와 전기진단검사와의 상관관계를 검정하기 위해 Pearson correlation test를 이용하였고 전기진단검사 중증도별 그룹에 따른 초음파 소견에 대한 분석은 Kruskal-Wallis test를 이용하였다. 모든 통계학적인 검정은 SPSS version 18.0 (SPSS Inc., Chicago, IL, USA)를 이용하여 유의수준 0.05에서 검정하였다.

결과

Fisher exact test를 이용하여 두 군 간의 성별, 나이, 양측성을 분석한 결과 두 군 간의 유의한 통계학적 차이는 없었다(Table 1). Independent t-test를 이용한 검정에서 두 군 간의 초음파 계측치에는 1구역에서의 CSA와 2구역 및 3구역의 압박률(FR) 등이 통계학적으로 의미 있는 차이를 보였다(Table 4, Fig. 3). 두 군의 측정치를 바탕으로 작성한 ROC curve (receiver operating characteristic curve)에서 cutoff value는 CSA=0.095 cm2, 민감도 70%, 특이도 53%; FR(Z2)=2.92, 민감도 73%, 특이도 73%; FR(Z3)=3.3, 민감도 67%, 특이도 67%였다(Fig. 4). 전기진단검사와 초음파 계측치의 상관관계를 확인하기 위해 시행한 Pearson correlation test에서는 1구역에서 D1과 NCV(F), D2와 TL 및 NCV(F), C와 NCV(F), CSA와 TL 및 NCV(F), 1구역과 2구역 면적비율(CSA ratio, Z1/Z2)과 TL이 통계학적으로 유의한 상관관계가 있는 것으로 확인되었으나 이에 대한 r값은 높지 못하여 강력한 선형관계를 갖지는 못했다(Table 5, Fig. 5). 전기진단검사에 의한 중증도별 분류에서 Steven 분류법 [11] 및 Bland Scale [12] 모두 각 분류군마다 유의한 초음파적 계측치 차이를 나타내지 못하였다(Kruskal-Wallis test를 이용한 검정에서 Bland Scale의 경우 1구역의 D2, 3구역의 D2, 3구역의 CSA가, Steven 분류법에서 CSA ratio (Z1/Z3)가 유의한 통계학적 차이가 있는 것으로 나왔지만 사후검정 Mann-Whitney test에서 유의한 통계학적 의미는 없었다)(Tables 6, 7).

고찰

전통적으로 수근관 증후군은 임상적 증상의 청취 및 이학적 검사 이후 시행한 전기진단검사의 소견으로 확진되었다[2]. 전기진단검사는 환자의 수술적 치료를 결정함에 있어서 근전도를 통한 무지구의 근위축이나 신경전도기능 이상의 중증도를 판단할 수 있을 뿐만 아니라 환자의 증상과도 연관성을 나타내는 것으로 알려져 있어 집도의의 명확한 판단에 큰 도움을 준다[2,12]. 문헌에 따라 차이가 있으나 임상의가 증상과 징후를 근거로 시행한 전기진단검사에는 91%-98% 양성을 보이는 것으로 되어 있는데 임상적 증상 및 징후가 확연하지 않을 경우 전기진단검사가 감별진단 및 수술적 치료 결정에 더 큰 도움을 줄 수 있다[10].

하지만 전기진단검사의 비용 및 소요시간, 환자가 검사 시 느끼는 불편감 등의 단점들이 있기 때문에 이를 극복하기 위하여 최근 초음파적 진단이 많이 시행되고 있는 추세이다. 수근관 증후군을 초음파로 진단하기 위하여 진단적 수치를 정량화하고 검증하는 연구는 일찍이 여러 저자에 의해 발표된 바가 있다[4-8,13-17]. 대부분 정중신경의 비대를 정량화하여 단면적을 진단에 이용하는데, 이러한 정중신경의 비대를 측정하는 위치 및 이러한 단면적 측정치에 대한 진단검사로서 민감도 및 특이도가 매우 상이하여 정확한 합의점이 없다는 것이 문제라 할 수 있다. 수근관 내부(pisiform level)의 정중신경 비대를 진단적 소견으로 제시한 연구도 있었으나 [3,13,15] 정중신경이 수근관에 들어가기 전 원위손목주름 혹은 방형회내근이 관찰되는 부분의 비대를 관찰한 연구들이 있으며 횡수근인대의 비대에 의한 압박이 핵심적인 병리임을 고려할 때 저자들의 연구결과와 같이 수근관 진입 직전의 정중신경의 직경을 측정하는 것이 정확한 방법이라 할 수 있겠다[8] (Fig. 6).

한편 FR 계측치는 2구역 및 3구역 모두에서 두 군 간에 통계학적으로 의미 있는 차이를 보였다. 다른 연구에서 FR는 원위 수근관에서 측정되어 3 이상의 값을 가질 때 진단적 가치가 있다는 주장을 한바 있지만 저자들의 ROC curve에서 확인한 바와 같이 2구역 혹은 3구역 모두 그리 높은 민감도 및 특이도를 가지지 못하여 반드시 어느 부분에서 측정해야만 진단이 가능하다 이야기할 수는 없다[3,9] (Fig. 4). 다만 3구역은 정상 해부학적 위치에서 정중신경이 수배부 쪽으로 굴곡되어 들어가는 형태로 초음파검사 시 비등방성(anisotropy)으로 인하여 측정이 힘들고 영상의 질이 떨어질 수 있으며 정상적으로 수지신경으로 분지되기 전의 형태로 납작한 모양을 보일 수 있다(3구역의 cutoff value가 2구역의 값보다 더 큰 것 역시 이를 설명해 줄 수 있다). 너무 원위부에서 측정 시 측정치의 오류가 있을 수 있으므로 2구역에서의 측정하는 것이 보다 정확할 것으로 사료된다.

많은 연구에서 수근관 증후군의 진단기준을 CSA=0.09 cm2 혹은 0.10 cm2으로 설정하고 있으며 일부 연구에서는 90% 이상의 민감도 및 특이도를 주장하기도 하였다[4,6,7,15,18,19]. 과거 연구에서 높은 민감도 및 특이도는 초음파 해상도의 문제로 인한 측정 오류에 의해 발생할 가능성도 배제할 수는 없지만 최근 2017년 Billakota와 Hobson-Webb [7]에 의한 연구에서 CSA의 민감도 및 특이도를 90% 이상이라 하였는데 이는 저자들의 연구결과와는 배치되는 결과이다. Fowler 등[20]은 2011년도 Meta-analysis를 통하여 민감도 및 특이도가 높지 못하고 결과적으로 전기진단검사를 대체하기 힘들다 발표한 바가 있고 현재 저자들의 연구에서도 높아진 초고해상도 탐촉자(extreme high density probe)를 이용했지만 비슷한 결과가 도출된 것으로 볼 때 수근관 증후군의 중증도 평가 및 침습적 처치 시행의 판단기준을 초음파적 계측치로 대체하는 것에는 한계가 있을 것으로 사료되며 이는 영상 계측의 방법이나 해상도의 문제가 아닌 질병 자체에서 신경변화의 폭이 다양한 것 때문으로 판단된다.

전기진단검사와 초음파 계측치와 연관성을 주장한 연구들이 있었다[7,8,17]. 하지만 본 연구에서 통계적으로는 선형연관관계를 가지고 있는 계측치들이 있었으나 r 값이 모두 0.5 미만으로 강력한 설명력을 갖지는 못하였고 전기진단검사를 기준으로한 중증도 분류법 역시 초음파 계측치와 연관성을 설명할 수는 없었다.

이 연구의 한계점으로 증례의 수가 많지 않다는 점과, 증상이 없는 대조군에서는 전기진단검사를 시행할 수 없었다는 점이 있었고, 무증상군부터 증상이 명확한 환자군까지 연속적인 대량의 증례에서 초음파 계측 및 전기진단검사가 이루어진다면 연관성에 대한 설명이 명확해질 수는 있겠으나 현실적으로 무증상군에서 대량의 전기진단검사를 시행하기는 힘들다고 할 수 있다.

결론

수근관 증후군의 진단에 있어서 해부학적 구역별로 확인해야 할 측정값은 각기 다른 것으로 확인되었으나 이러한 검사치들은 민감도 및 특이도가 낮아 단독으로 진단하기에는 무리가 있을 수 있으며, 증상 및 근전도를 이용한 고식적인 방법에 추가적인 진단 및 술기의 보조적, 보완적 수단으로 적절할 것으로 판단된다.
