Trans Abstract

Purpose

An occult surgical neck fracture can be accompanied in isolated greater tuberosity fracture of humerus. The risk factors of occult surgical neck fracture in isolated greater tuberosity fracture were statistically analyzed.

Methods

From 2008 to 2017, 74 patients with isolated greater tuberosity fracture of humerus were selected. The occult surgical neck fracture was evaluated by computed tomography or magnetic resonance imaging. Patients were divided into two groups, those with isolated greater tuberosity fractures (group I) and isolated greater tuberosity fractures with occult surgical neck fracture (group II). For the risk factor analysis, the age, sex, underlying disease, injury mechanism, and body mass index between the two groups were compared. Radiologically, fracture pattern, the amount of displacement, and the presence and direction of the shoulder joint dislocation were compared.

Results

In multivariate analysis, factors independently related to the occurrence of occult surgical neck fractures were gender and whether or not the joint dislocation. In Female (odds ratio [OR]=14.806; 95% confidence interval [CI], 1.188–184.503; p=0.036) and in cases where no joint dislocation occult surgical neck fracture (OR=0.018; 95% CI, 0.001–0.271; p=0.004) was accompanied more frequently and occurred statistically.

Conclusion

Care must be taken in cases of female and the absence of shoulder dislocation in isolated greater tuberosity fracture because of the possibility of occult surgical neck fracture.

서론

상완골 대결절 골절은 회전근개에 의한 견열 골절로 발생하거나 견봉 또는 관절 와에 상완골두가 충돌하여 발생하는 것으로 알려져 있다[1,2]. 상완골 대결절 골절 시 일부에서 외과적 경부에 잠행 골절이 동반되는데 Shaw 등[3]은 약 11.8%로 보고하였다. 상완골 대결절 골절에 외과적 경부 잠행 골절이 동반된 경우 대결절 골절만 있는 경우와 비교하여 외과적 경부 잠행 골절이 진성 골절로 진행할 수 있어 주의가 필요하나[3-6] 일반 방사선검사만으로는 외과적 경부 잠행 골절의 진단이 어려워 추가적인 검사가 필요한 경우가 많다. 상완골 대결절 골절 시 외과적 경부 잠행 골절이 발생하는 위험인자를 알 수 있다면 이의 예측 및 조기 진단에 도움이 될 수 있을 것이다. 하지만, 상완골 대결절 골절 시 외과적 경부 잠행 골절이 발생하는 위험인자는 정확히 알려져 있지 않다.

이에 저자들은 상완골 대결절 골절 시 외과적 경부 잠행 골절이 발생하는 위험인자를 분석하고자 본 연구를 시행하였다.

대상 및 방법

2008년부터 2017년까지 본원에 상완골 대결절 단독 골절로 내원한 20세 이상의 환자 88명 중 병적 골절 및 근위 상완골 골절의 과거력이 있는 경우는 제외하였고 컴퓨터단층촬영(computed tomography, CT) 또는 자기공명영상(magnetic resonance imaging, MRI) 검사를 시행한 74명을 대상으로 하였다. 일반 방사선검사에서는 74명의 환자 모두에서 상완골 대결절 골절만이 확인되었다. 하지만, 골절의 양상 및 동반 손상의 여부를 확인하기 위해 시행한 CT나 MRI에서 일반 방사선검사에서는 관찰되지 않았던 골절선이 발견된 경우가 있었다. 이와 같이 일반 방사선검사에서는 보이지 않으나 CT 또는 MRI상 골절선이 관찰되는 경우 이를 잠행 골절로 정의하였다. CT를 시행한 경우가 9명, MRI를 시행한 경우가 64명, CT와 MRI를 모두 시행한 경우가 1명이었다.

상완골 대결절에만 골절이 있었던 경우를 Ⅰ군(대결절 단독 골절군)으로 분류하였고(Fig. 1), 상완골 대결절 골절에 외과적 경부 잠행 골절이 동반된 군을 Ⅱ군(외과적 경부 잠행 골절군)으로 분류하였다(Fig. 2). 위험인자 분석을 위해서 두 군 간 나이, 성별, 기저질환, 수상 기전, 체질량 지수(body mass index, BMI)를 비교 분석하였다. 기저질환은 고혈압 및 당뇨 여부를 분석하였으며 BMI는 25 kg/m2를 기준으로 평가하였다. 방사선학적으로 상완골 대결절 골절 형태를 Mutch 등[7]이 제안한 견열형(avulsion type), 분리형(split type), 감입형(depression type)으로 분류하였고, 골절편의 전위 정도, 견관절 탈구 유무 및 방향을 비교분석하였다.

통계분석은 SPSS ver. 19.0 (IBM Co., Armonk, NY, USA)을 이용하였다. t-test 및 chi-squared test를 이용하여 분석하였으며 단변량분석 및 다변량분석을 유의수준 0.05에서 실시하였다.

본 연구는 가톨릭대학교병원 연구윤리위원회의 승인을 받았다(IRB No. 2019-4327-0001).

결과

1. 환자의 특성

Ⅰ군이 57명(77.0%), Ⅱ군이 17명(23.0%)으로 분류되었다. 전체 74명 환자의 평균나이는 57.5±13.1세(범위, 24-87세)였으며 여자가 48명(64.9%)으로 많았다. 골절의 발생 원인으로는 넘어진 경우가 62명(83.8%)으로 가장 많았으며 그 밖에 보행자 교통사고(4명, 5.4%), 승객 교통사고(2명, 2.7%), 오토바이 사고(2명, 2.7%) 등이 있었다. 고혈압과 당뇨를 동반하고 있는 환자는 각각 25명(33.8%)과 12명(16.2%)이었으며 이 중 10명의 환자는 고혈압과 당뇨를 모두 가지고 있었다. 환자의 평균 BMI는 24.37±3.60 kg/m2 (범위, 16.65-35.06 kg/m2)이었고 BMI 값이 25 이상으로 비만에 해당하는 환자는 26명(35.1%)이었다. 대결절 골절의 형태는 견열형이 52명(70.3%), 분리형이 19명(25.7%), 감입형이 3명(4.1%) 순이었고, 대결절 골절편의 평균 전위는 8.45±4.15 mm (범위, 2-20 mm)였다. 견관절 탈구는 38명(51.4%)의 환자에서 발생하였으며 모두 전방 탈구였다(Table 1).

2. 단변량분석(univariate analysis)

단변량분석에서 I군과 II군의 평균연령은 차이가 없었으나(p=0.971) 성별의 경우 여성의 비율이 I군에서는 57.9%, II군에서는 88.2%로 유의하게 II군에서 여성이 많았다(p=0.021). I군의 수상 기전은 넘어진 경우가 45례(78.9%), 교통사고 관련된 경우가 12례(21.1%)였으며 II군의 경우 모두 넘어져서 수상하였다(p=0.039). 고혈압과 당뇨 동반 여부는 두 군 간의 차이를 보이지 않았다. BMI는 II군에서 25.98±3.79 kg/m2로 I군 23.89±3.44 kg/m2보다 유의하게 높았다(p=0.034). 대결절 골절의 형태는 I군의 경우 견열형이 75.4%로 가장 많았으며 II군에서는 견열형이 52.9%, 분리형이 47.1%였으나 통계적인 차이는 없었다(p=0.056). 대결절 골절편의 평균전위는 I군에서 8.49±4.27 mm, II군에서 8.29±3.85 mm로 차이를 보이지 않았다(p=0.865). 견관절 탈구는 I군 57명 중 37명(64.9%)에서 발생하였으며 II군에서는 17명 중 1명(5.9%)에서 발생하여 I군에서 유의하게(p<0.001) 견관절 탈구가 더 많이 동반되었다(Table 2).

3. 다변량분석(multivariate analysis)

단변량분석에서 p값이 0.1 미만을 보이는 성별, 수상 기전, BMI, 대결절 골절의 형태, 견관절 탈구의 여부로 다변량분석을 시행하였다. 결과적으로 다변량분석에서 외과적 경부 잠행 골절의 발생과 독립적으로 관련이 있는 인자는 성별과 견관절 탈구의 여부였다. 즉, 여성(odds ratio [OR]=14.806; 95% confidence interval [CI], 1.188-184.503; p=0.036)에서, 그리고 견관절 탈구(OR=0.018; 95% CI, 0.001-0.271; p=0.004)가 동반되지 않은 경우에서 외과적 경부 잠행 골절이 더 많이 발생하였다(Table 3).

고찰

상완골 대결절 골절은 상완골 근위부 골절의 약 14%-21%를 차지한다[8,9]. 상완골 대결절 골절 시 일부에서 외과적 경부에 잠행 골절이 발생하는 것으로 보고되고 있다[3,10]. 상완골 대결절 골절에 외과적 경부 잠행 골절이 동반된 경우는 대결절 단독 골절만 있는 경우와 비교하여 긴 고정 기간이 필요하며[3-6] 외과적 경부 잠행 골절이 진성 골절로 진행하면서 전위나 각 형성이 발생할 수 있으므로 주의를 요한다[11-13]. 하지만, 현재까지 상완골 대결절 골절 시 외과적 경부에 잠행 골절이 발생하는 위험인자에 대해서는 명확하게 밝혀진 바가 없으며, 이러한 이유로 저자들은 본 연구를 시행하였다.

상완골 대결절 골절에 동반된 외과적 경부 잠행 골절은 조기에 진단이 어려운 경우가 많다. Shaw 등[3]은 상완골 대결절 단독 골절환자 68명 중 8명(11.8%)에서 외과적 경부 잠행 골절이 발생하였으며 이 중 3명은 초기 진단에서 발견하지 못하였다고 보고하였다. Wu 등[10]은 상완골 대결절 골절과 동반된 외과적 경부 잠행 골절의 초기 진단을 놓치는 이유는 통증으로 인하여 정확한 각도에서 일반 방사선 촬영이 어려운 경우가 많고, 외과적 경부 잠행 골절 발생에 대한 지식 및 경험 부족으로 이를 예측하지 못하기 때문이라고 설명하였다. 본 연구에서도 모든 증례에서 외과적 경부 잠행 골절은 CT 또는 MRI를 통하여 확인이 가능하였다. 특히, MRI의 경우 신호 강도의 변화가 뚜렷하여 외과적 경부 잠행 골절의 진단에 유용하였다. Haubro 등[14]은 대퇴골 근위부 잠행 골절의 진단에서 MRI가 CT보다 민감도(sensitivity)가 높아 진단이 용이하다고 보고하였고, Gumina 등[15]도 상완골 근위부 잠행 골절의 진단에 MRI가 매우 유용하다는 점을 보고한 바 있다. 따라서, 상완골 대결절 골절 시 외과적 경부 잠행 골절이 발생할 수 있는 위험군의 경우 CT나 MRI를 추가적으로 시행하여 이를 평가하는 것이 필요할 것으로 생각된다.

상완골 단독 대결절 골절 발생과 연관이 있을 것으로 생각되는 위험인자에 대한 연구는 아직까지 정확히 밝혀져 있지 않다. Cuomo [16]와 Flatow 등[17]은 골다공증 및 외과적 경부의 약한 골소주 구조가 외과적 경부 골절에 영향을 줄 수 있다고 보고하였다. 본 연구에서는 여성에서 외과적 경부 잠행 골절의 발생빈도가 유의하게 증가하였다. 이는 여성에서 골다공증의 유병률이 높은 것과 관련이 있을 것으로 생각된다. 저자들도 골다공증이 외과적 경부 잠행 골절의 위험인자일 것으로 예상하고 이를 분석하였으나 아쉽게도 골밀도검사(bone marrow density, BMD)를 시행한 증례의 수가 적어 통계적으로 유의한 결과를 얻지 못하였다. 상완골 대결절 골절 시 동반되는 외과적 경부 잠행 골절과 골다공증과의 연관성에 대해 추가적인 연구가 필요할 것으로 생각한다.

상완골 대결절 골절 시 견관절 탈구 유무도 외과적 경부 잠행 골절의 발생과 관련이 있는 것으로 판단되었다. 즉, 견관절 탈구가 발생하지 않은 상완골 대결절 골절에서 외과적 경부 잠행 골절이 유의하게 증가하였다. I군(대결절 단독 골절군)은 57명 중 37명(64.9%)에서 견관절 탈구가 발생하였으나, Ⅱ군(외과적 경부 잠행 골절군)은 17명 중 1명에서만 견관절 탈구가 발생하였다. 일반적으로 상완골 대결절 골절 시 15%-30%에서 견관절 탈구가 발생하는 것으로 보고되었으나[18], 견관절 탈구와 외과적 경부 잠행 골절의 관련성에 대해서는 명확하게 보고된 바가 없다. 저자들은 견관절 탈구가 발생하지 않은 상완골 대결절 골절에서 외과적 경부 잠행 골절이 증가하는 이유가 수상 시 외력의 작용 과정과 관련이 있을 것으로 추측하였다. 대결절 골절 시 탈구가 동반되는 경우에는 외력이 탈구에 작용하므로 외과적 경부에 가해지는 손상이 적으나, 탈구가 발생하지 않는 경우에는 외력이 구조적으로 약한 부위인 외과적 경부에 작용하여 잠행 골절이 발생하는 것으로 생각된다.

상완골 대결절 골절 형태와 외과적 경부 잠행 골절의 관련성을 분석한 결과 Ⅰ군의 경우 견열형이 75.4%로 가장 많았으며 Ⅱ군에서는 견열형이 52.9%, 분리형이 47.1%였으나 두 군 간 통계적으로 유의한 차이는 없었다(p=0.056). Shaw 등[3]도 분리형에서 외과적 경부 잠행 골절이 많이 발생하였으나 통계적인 유의성은 없었다고 보고하였다. 더 많은 수의 증례를 통한 상완골 대결절 골절 형태와 외과적 경부 잠행 골절 관련성에 대한 연구가 필요할 것으로 생각된다.

이 연구에는 몇 가지 제한점이 있다. 첫째, 후향적 연구이며 증례의 수가 적다는 점이다. 둘째, 추가적인 검사로 CT 또는 MRI를 시행하여 균일한 영상학적 분석에 차이가 발생할 가능성이 있다. 셋째, 영상학적 분석에 대한 관찰자 간 신뢰도(interobserver reliability) 차이가 발생할 수 있다. 넷째, BMD를 시행한 증례가 적어 외과적 경부 잠행 골절과 골다공증의 관련성을 입증하지 못하였다는 점이다.

결론

상완골 대결절 단독 골절 시 여성이며 탈구가 발생하지 않은 경우 외과적 경부 잠행 골절의 발생 가능성이 높으므로 주의를 요한다.
