Trans Abstract

Acro-osteolysis is a bone resorption reaction that progresses slowly in the distal phalanx of the hand and foot and is associated with various diseases. It can be classified as idiopathic or secondary. Although the mechanism of acro-osteolysis has not been fully elucidated, the presence of a chronic ischemic injury appears to have a significant effect, and bone metabolism dysregulation due to the accompanying calcinosis or peripheral neuropathy also appears to contribute. Acro-osteolysis can show various clinical and radiological features, and the differential diagnosis of the underlying etiology is essential. It is a rare and sporadic disease worldwide. The authors experienced a case of acro-osteolysis suspected of idiopathic cause in a Black woman; therefore, we report this case with a literature review.

서론

말단골용해(acro-osteolysis)는 손과 발의 원위지골에서 서서히 진행하는 골흡수(bone resorption) 반응으로 다양한 질환들과 연관되어 발생하며, 원인질환이 없는 특발성과 독성물질 노출, 류마티스 질환, 감염성 질환, 대사성 질환, 유전질환 등에 의한 이차성으로 구분된다[1]. 골흡수를 유발하는 정확한 기전과 병인은 아직 불분명하며 수지 원위부의 혈류 순환장애, 석회화, 말초신경병증 등과 연관이 있을 것으로 추정된다[1,2].

진단을 위해 단순 방사선촬영이 가장 많이 사용되고 있으나, 초음파와 자기공명영상 등의 영상검사도 진단에 도움을 줄 수 있다[1]. 말단골용해는 보통 원위지골의 기저부는 유지된 채로 골흡수가 일어나며 영상의학적으로 두 가지 패턴으로 나뉘어지는데, 더 흔하고 일반적인 유형으로 원위지골 말단부에서부터 흡수가 일어나는 말단형(tuft type)과, 원위지골 말단부가 남아있는 상태에서 중간부에서 횡형으로 골흡수가 발생하는 밴드형(band type)이 있다[1,2]. 말단골용해를 유발하는 기저 질환에 따라 영상의학적으로 나타나는 특징이 다르다고 알려져 있으며, 말단형은 전신 경화증, 부갑상선기능항진증, 말초신경병증에서 더 흔히 동반되고 밴드형은 염화비닐 노출, 건선 관절염, 감염성 질환에서 더 많이 보인다(Table 1) [1,2].

말단골용해는 전 세계적으로 매우 드문 질환으로 문헌 검색에서도 산발적인 증례 보고들이 주를 이루며 국내에서는 보고가 거의 없는 상태로, 특히 특발성으로 발생한 경우는 없었다. 저자들은 흑인 여성에서 특발성으로 발생한 것이 의심되는 말단골용해 환자를 경험하였기에 이를 문헌 고찰과 함께 보고하는 바이다.

증례

34세 흑인 여성이 1년간 지속된 좌측 제2수지 원위지의 통증과 붓기로 본원 정형외과 외래에 내원하였다. 최근 3개월 동안 손가락 끝의 통증과 붓기가 빠르게 악화되어 점차 손가락을 사용하기 어려워졌다. 환자는 한국에서 복무 중인 미군의 가족이며 비정기적으로 영어를 가르치는 직업을 가지고 있었다. 직업적으로 손을 많이 사용하거나 반복적인 진동 또는 폴리염화비닐에 노출된 직업력은 없었다. 또한 류마티스 질환, 감염, 내분비질환 등의 과거력도 없었으며 동상, 화상 등을 포함하여 수지 외상 과거력, 유전질환이나 선천성 골형성이상, 건선 등의 가족력도 없었다. 환자 외관상 급성 병색은 뚜렷하지 않았고 163 cm의 키에 52 kg의 몸무게로 전형적인 흑인처럼 눈썹이 짙고 곱슬머리에 머리숱이 많았으며, 타원형의 두상에 미간이 넓고 편평하였으나 후두골 돌출 소견은 없었다. 청력과 지능도 정상이었다.

신체검사에서 시진상 좌측 제2수지의 말단부가 둥글고 뭉툭한 곤봉지 소견을 보였고 손톱의 단축변형이 진행된 상태였다. 원위지간관절의 가동범위는 신전 0°–굴곡 60°였고 원위지간관절의 변형이나 과운동성은 관찰되지 않았다. 수지의 종창이나 발적과 같이 감염증을 시사하는 소견은 없었다. 찬물에 손을 오래 담그기 어렵다고 진술하였으나 피부 색깔이 붉어지면서 통증이 악화되는 전형적인 레이노 현상(Raynaud phenomenon)은 보이지 않았으며 이환된 수지에서 측정한 동맥혈산소포화도는 97%였다.

좌측 수부 단순 방사선촬영에서 제2수지 원위지골에서 골성 단편과 경계가 불명확한 골흡수 소견을 보였다(Fig. 1). 사구종 등 조갑판에 발생할 수 있는 병변을 감별하기 위해 추가적인 좌측 수부 자기공명영상검사를 시행한 결과 종괴 같은 공간 점유성 병변은 없었으며, 골흡수 소견과 함께 골 부종과 미란을 보였고 주변 연부조직들의 신호가 증가되어 있었다(Fig. 2). 척추의 단순 방사선촬영상 흉-요추에 압박골절 등 특별한 병적 소견은 없었으며 골밀도 검사에서도 요추 1–4번 부위 평균 T-점수가 1.5로 정상 소견을 보였다. 심전도와 흉부 단순 방사선촬영은 정상이었다.

혈액검사에서 백혈구 수가 3,700/mm3였고, 적혈구 침강속도와 C-반응성 단백도가 각각 8 mm/hr (상한치, 20 mm/hr)과 0.02 mg/dL (상한치, 0.5 mg/dL)로 정상범위 내였으며, 항핵항체(anti-nucleus antibody) 검사와 류마티스 인자(rheumatoid factor) 검사는 음성이었다. 당화혈색소, 간기능검사, 신기능검사, 갑상샘기능검사에서도 정상 소견을 보였다. 정확한 진단을 위하여 추가적인 혈액검사와 조직검사를 권유하였으나 환자의 개인 사정으로 추적 소실되었으며, 3년 뒤 다시 외래에 내원하였다.

3년 사이에 좌측 제2수지의 곤봉지가 심화되었을 뿐만 아니라 우측 제1, 2, 3수지에서도 같은 변화를 보였다(Fig. 3). 역시 수지의 종창이나 발적과 같이 감염증을 시사하는 소견은 없었고 혈액검사 소견도 정상이었다. 원위지간관절 이하로 변형이 진행되지는 않았다. 양측 수부 단순 방사선촬영에서 좌측 제2수지와 우측 제1, 2, 3수지 원위지골에서 밴드형 말단골용해가 진행된 소견을 보였다(Fig. 4). 이후 환자는 통증 조절과 골용해를 막기 위해 스테로이드와 비스포스포네이트(bisphosphonate)를 경구 투약하였으며 추가적인 증상의 악화 없이 경과관찰 중이다.

본 증례보고는 임상 사진에 게재에 대한 환자의 동의를 받았다.

고찰

말단골용해는 다양한 기저 질환과 외부 요인에 의해 발생할 수 있으며 대표적인 원인들은 PINCH FO라는 약어로 축약된다(P: psoriasis/pyknodysostosis; I: injury, e.g. thermal burn, frost bite; N: neuropathy, e.g. diabetes mellitus, leprosy; C: collagen vascular disease, e.g. scleroderma, Raynaud disease; H: hyperparathyroidism; F: familial, e.g. Hajdu-Cheney syndrome, O: others, e.g. polyvinyl chloride exposure, progeria acronyms). 이처럼 말단골용해가 발생하는 원인은 다양하며 구체적인 병인은 아직 불분명하지만 현재까지는 수지 원위부의 만성적인 허혈성 손상이 가장 큰 영향을 미치는 것으로 알려져 있으며, 이외에도 동반된 말초신경병증에 의한 골 대사조절 장애 등이 기여하는 것으로 보인다[1,2]. 말단골용해는 드물지만 다양한 임상적, 영상의학적 양상을 나타낼 수 있으며 원인이 되는 기저 병인에 대한 감별 진단이 중요하다. 유전적인 요인에 의한 말단골용해는 보통 10대에 처음 발생하게 되는 반면, 특발성으로 생기는 경우는 30대 중반 이후에 처음 발생한다고 알려져 있다[2].

본 환자의 경우 발생 연령과 직업 및 가족력을 고려하였을 때 독성물질 노출이나 유전성 질환의 가능성을 배제할 수 있으며, 혈액검사 결과 감염증이나 내분비질환에 의한 것도 아니었다. 건선의 가족력이 없고 특징적인 건선형 손톱 모양과 지절염(dactylitis) 소견이 없는 상태로 건선 관절염에도 해당하지 않으며, 류마티스 병력과 검사상 전신경화증 진단기준에도 부합하지 않는다[3,4]. 특히 전신경화증의 경우 이환 기간이 길고 질병 활성도가 높은 경우에 말단골용해가 발생하며, 말단형 형태로 발생하는 것으로 알려져 있다[5]. 따라서 본 환자는 특발성으로 발생한 경우일 가능성이 크다고 생각되나, 최근 전신경화증의 첫 증상으로 말단골용해가 나타날 수 있다고 보고한 문헌도 있어 향후 환자의 경과에 따라 진단이 바뀔 수 있으므로 좀더 장기적인 추시가 필요하다[6].

저자들의 문헌 검색에 의하면 말단골용해는 그 희소성으로 발생률이 직접 조사된 문헌은 없었다. 다만 전신경화증을 가진 환자에서 말단골용해의 발생빈도를 관찰한 연구에서 120명의 전신경화증 환자 중 77명에서(64.1%) 발생했으며, 성별과 나이에 따라서 통계적으로 발생률이 다르지 않았다고 보고하였다[6]. 국내에서 지금까지 보고된 말단골용해 증례 보고는 총 3예며, 보고된 증례들을 표로 요약하여 정리하였다(Table 2) [7-9]. 전신경화증에 의한 말단골용해 2예와 Hajdu-Cheney syndrome 증후군에 의한 1예가 있었으며 특발성으로 발생한 경우는 없었다. 전신경화증에 의한 경우는 모두 여성이었고 레이노 현상을 동반한 말단형으로 나타났다[7,9]. 말단골용해와 동반된 생인손이나 석회화가 있는 경우 수술적 치료를 시행한 뒤 호전되었다[7,9]. 이번 환자는 비록 한국인은 아니지만 특이 과거력 없이 발생한 밴드형 말단골용해가 3년 사이 빠르게 진행하여 양손의 수지를 다발성으로 침범한 특발성이 의심되는 드문 증례이다.

Characteristics of patients with acro-osteolysis reported in previous studies

특발성 말단골용해의 특별한 치료는 없으며 증상에 따라 비스테로이드성 소염진통제 및 스테로이드, 항섬유화제인 페니실라민(penicillamine)이나 콜히친, 항혈소판제, 면역억제제, 비스포스포네이트 등을 사용해 볼 수 있으나, 아직 골흡수를 막는 근본적인 치료 방법은 정립되지 않은 상태이다[2,10]. 약물에 반응하지 않는 환자에서 교감신경 절제술 또는 말단부 지방 이식술 등이 수술적 치료로 적용된 보고들이 있으나 수술 후 역으로 말단골용해가 급격하게 진행할 가능성도 있어, 충분한 보존적 치료를 거친 뒤 기능적 측면에서 환자와 긴밀하게 상의한 뒤에 제한적으로 시행하여야 한다[10].

말단골용해는 손과 발의 원위지골에서 서서히 진행하는 골흡수 반응으로 다양한 질환들과 연관되어 발생하며, 원인을 알 수 없는 원발성과 기저 질환에 의한 이차성으로 구분할 수 있다. 말단골용해는 다양한 임상적, 영상의학적 양상을 나타낼 수 있으며 원인이 되는 기저 병인에 대한 감별 진단이 중요하다. 말단골용해는 전 세계적으로 매우 드문 질환으로 저자들은 특발성으로 발생한 것으로 의심되는 흑인 여성 말단골용해 환자를 경험하였기에 이를 문헌 고찰과 함께 보고한다.
