Carpal Tunnel Release with Minimal Single Palmar Incision

Abstract
Purpose
To analyze the clinical results of the minimal single palmar incision carpal tunnel release. Materials and Methods: The clinical results of 105 wrists of 68 patients, who underwent the minimal single palmar incision carpal tunnel release during the period of March 1997 and December 2000, were analyzed. The average follow-up period was 16.5 months (range, 6-35 months). There were 6 male (8 wrists) and 62 female (97 wrists) patients. The average age was 54.7 years (range, 21-81 years). The basic demographic analysis based on the medical records was done and the outcomes of surgical treatment were assessed by the Cseuz’s criteria. Results: Most of cases were over their 60’s (73.3%) and 83 cases (79%) were idiopathic. Other underlying causes were rheumatoid arthritis in 12 cases, diabetes mellitus in 4 cases, distal radius fracture in 4 cases, chronic renal failure in 1 case and hypothyroidism in 1 case. In the preoperative electrodiagnostic study, all cases showed abnormal nerve function. Seventy three cases accompanied thenar muscle atrophy. At the last follow up, the results based on the Cseuz’s criteria were excellent in 68 cases (64.8%), good in 26 cases (24.7%), fair in 3 cases (2.9%) and poor in 8 cases (7.6%). Night pain disappeared within 3 days (61/63 cases, 96.8%). Sensory dysfunction and thenar muscle atrophy were recovered in most cases except 8 cases who suffered from carpal tunnel syndrome for more than 5 years. No complication such as neurovascular injury occurred. Conclusions: The minimal single palmar incision carpal tunnel release provides reliable, effective results, which is comparable to the endoscopic and percutaneous procedures. This procedure don’t need any specialized tool, but is easy to learn and provide good visual operative field.
