Abstract |
Object: We wanted to know the results and usefulness of surgery in total elbow replacement using Kudo system in patients with rheumatoid arthritis of elbow. Patients and Methods: We surveyed 33 cases (31 patients), who were treated with rheumatoid arthritis of elbows and had operated total elbow replacement with Kudo type III system from November, 1996 to September, 2003. The average age of the patients was 46.1 years and prevalance was 7.1 years and average follow up period was 46.5 months. We analysed MAYO elbow performance index (MEPI). We have done physical examination and radiographic examination before and after the surgery, which proceeded extended koch approach with Kudo type III system. After the surgery, we applied long arm splint. If there was no problem in operation site, we proceeded elbow flexion and extension exercise in beginning with active motion with gentle passive assisted motion at 3rd week after operation. Result: Pain was relieved in 32 cases of 33 cases(97.0%). Mean flexion and extension range of motion was improved from extension 41 degree and flexion 91 degree to extension 39 degree and flexion 111 degree. Average MEPI score was 74.8 with 8(24%) excellent, 16(49%) good, 6(18%) fair and 3(9%) poor cases. We found osteolysis around the distal humerus components in 2 cases in radiographic follow up, but there was no pain and instability. Conclusion: Total elbow replacement with Kudo type III system in rheumatoid arthritis of elbow was result of pain relief and promotion of range of motion. But there were needed to confirm states of prosthesis and patients in long term follow up. |
