Multiple Carpometacarpal Dislocation associated with Scaphoid Fracture - A Case Report -

Abstract
Multiple carpometacarpal dislocation are rare and are usually the results of injuries of greater energy than those causing isolated dislocation and almost always need open reduction. Dorsal 2,3,4 carpometacarpal dislocation associated with scaphoid waist fracture has not been reported until now. We treated a case of multiple carpometacarpal dislocation associated with scaphoid waist fracture in 40-year-old male patient with open reduction and internal fixation with multiple Kirschner’s wire. After postoperative 8 weeks, bony resorption was observed at the fracture site, so we treated by corticocancellous wedge iliac bone graft with Acutrac and K-wire. But avascular necrosis of the proximal pole occurred with a scaphoid waist nonunion later, so we treated by vascularized bone grafting from the dorsal radius. We report upon this case with a brief review of the literature.
