POST-TRAUMATIC VERSUS IDIOPATHIC STIFF ELBOW AFTER OPEN ARTHROLYSIS : COMPARISON OF TWO PAIRED SERIES |

Abstract |
Purpose : The purpose of this study was to compare the long-term results of postoperative range of motion (ROM) with matched paired prospective test between post-traumatic stiff elbow and idiopathic stiff elbow after open arthrolysis for limitation of motion(LOM) of the elbow. Materials and Methods : A prospective study was conducted that evaluated the results of open arthrolysis between February 1994 and March 1998 by 1 surgeon. Thirteen of those had post-traumatic stiffness of elbow and the others had no trauma or infection history in the elbow. Twelve patients had intra-articular destructive change seen on operation that involved more than 30% of articular surface (8 cases in idiopathic group and 4 cases in post-traumatic group). Results : The total arc of motion in post-traumatic group improved from a preoperative mean of 50.8˚ to a postoperative mean of 110˚. In the idiopathic group, the mean arc was 54.2˚ and increased up to 100.8˚ postoperatively. Twelve patients who had intra-articular destructive change more than 30% (arthrosis group) gained little postoperative ROM after arthrolysis than non-arthrosis group (99.6˚, 110.4˚ respectively) and flexion contracure remained more (18.8˚, 11.4˚ respectively). Conclusion:Patients with post-traumatic stiffness achieved more ROM in postoperative total range of motion than idiopathic stiff group. Idiopathic group had a history of overuse of elbow joint and evidence of diffuse articular surface irregularity or denudation on operation. The patients with arthrosis gained less range of motion and have more flexion contracture after soft tissue contractures was all removed with open arthrolysis. |
