TARDY ULNAR NERVE PALSY WITHOUT CUBITUS VALGUS DEFORMITY

Abstract
Tardy ulnar nerve palsy typically results from cubitus valgus caused by the fracture of lateral condyle of the humerus during the growing period. But the cubital tunnel syndrome has many other etiologic factors. The purpose of this study is to report the many causes of the cubital tunnel syndrome by analyzing authors' cases which were diagnosed as tardy ulnar nerve palsy. From June 1996 to July 1999, we reviewed forty seven patients who were diagnosed as the ulnar nerve neuropathy at elbow by the clinical symptoms and electromyography. The cases of ulnar nerve injury which occured immediately after trauma or operation were excluded. The cases were classified with severity of symptoms by Osborne's criteria. In twenty cases, surgical treatments including detection and removal of the cause of compression were performed and postoperative results were evaluated with Messina's classification. The twenty six cases of thirty eight cases which were evaluated by X-ray have abnormal findings, but only six cases of typical cubitus valgus deformity were found. In operative findings, there were four cases of ganglion, two cases of ulnar nerve dislocation, two cases of hard and thickened medial triceps. The results of operation were correlated with preoperative severity of symptoms. In case of being suspicious of cubital tunnel syndrome without cubitus valgus, we think surgical exploration is highly recommended in consideration of many etiologic factors of cubital tunnel syndrome in mind.
