OPERATIVE MANAGEMENT FOR COMMINUTED FRACTURES OF THE DISTAL RADIUS

Abstract
Purpose
To analyze the clinical and radiological results according to treatment modalities for treating comminuted distal radius fractures. Materials and Methods: We reviewed 25 comminuted distal radius fractures in 24 patients who had been followed up for at least one year after operative management. Operative management included closed reduction and percutaneous pinning or external fixation, open reduction and internal fixation and open reduction, internal fixation augmented by external fixation. The clinical results were analyzed based on the modified Greens and OBrien functional scoring system. The radiological findings were evaluated by measuring the radial inclination, volar tilt and intra-articular step off. Results: In the last follow-up examination, the mean loss of radial length, volar tilt and intra-articular step off tended to be less in open reductions group than closed reduction group. The clinical results demonstrated that the open reduction group showed greater functional scores than closed reduction group. Conclusion: Surgical management such as open reduction, internal fixation, combined external fixation or bone graft which should be enough to reduce and maintain comminuted distal radius fractures are recommended.
