Availability of Intrasound Vibratory Apparatus in Diagnosis of Carpal Scaphoid Fracture

Abstract
Fracture of the scaphoid is the most common fracture of the carpus and most commonly undiagnosed fracture of the upper extremity beasuse its fracture is undisplaced or patient's symptom is not prominant. So we examed the both wrists of 60 patients with intrasound vibratory apparatus at both anatomical snuff box who suffered the suspicious acute wrist injury to confirm the availability of this apparatus. Then we devided the three groups of results that the first group was no different on both wrists, the second group had discomfort sensation at affected side of wrist and the third group had definite pain on affected wrist. In 60 patients, 26 patients in the first group, 24 patients in the second group and 10 patients in the third group. The diagnosis of fracture by X-ray were 8 patients, 3 patients in the second group and 5 patients in the third group. The another patients except 3 patients in the second group, 21 patients did't have scaphoid fracture. Only one patient in the third group, who was not found in initial X-ray and then two weeks later, made diagnosis of scaphoid fracture by X-ray and in this case we conclusded missdiagnosis case by reviewed film. In this study, the sensitivity of intrasound vibraory apparatus was 100%, but specificity was only 51% and positive predictable value was 26.5% and negative predictable value was 100%. It meant that had many false positive patients(25/34). But we thought that intrasound vibratory examination was useful screening test of wrist injuries, especially the scaphoid fracture because which apparatus was inexpensive, noninvasive, and easy to perform, and involves no ionizing radiation.
