Abstract |
Pullout wire suture technique is one of the most useful and popular fixating technique in the hand surgery such as mallet finger, distal tendon injuries and joint capsule and collateral ligament repair or reconstruction. Author introduce the more convenient technique for wire passing technique in pullout wire suture technique. After drilling of the bone for passing the fixating wire, insert the hypodermic needle directed to the outer skin through the drill hole which made by 0.046 inch Kirschner wire. No.26 wire (0.018 inch) introduced to the inner hole of that hypodermic needle through the drilled bone and soft tissue and skin which covered bone. Another same sized hypodermic needle introduced to proximal direction along the N.26 wire that penetrated first hypodermic needle until the both tips of the hypodermic needles those are directed opposite side. With rotating the needle, side cut of those needles adjusted to forming closed long tube which contains wire in the tube. The closed tube which formed with two needles centered with wire can pass easily through skin, soft tissues and bone without resistance. After removal of the first needle and inner wire, pullout wire can pass very easily through the second hypodermic needle to the outer surface of the skin. Repeat that procedure for pullout wire suturing on the skin with or without button for securing the pullout wire tying. The main advantages of this proposed technique is simplicity and easiness. Another advantage is larger wire can be pulled out through the smaller drill hole with this procedure that avoid larger drill hole in the bone which will be made in straight suture needle aid passing technique that is popular technique in the pullout wire suture. |
