Diagnosis of Carpal Tunnel Syndrome in MRI

Abstract
The magnetic resonance images of wrist in 9 CTS patients and 10 normal volunteers were studied. Five findings, cross-sectional area, swelling ratio and flattening ratio of median nerve, palmar deviation and bowing ratio of transverse carpal ligament were evaluated at the level of distal radio-ulna joint, pisiform and hook of hamate. All values were calculated and analyzed with computer using Image-Pro Plus program. Cross-sectional area and swelling ratio of median nerve were markedly reduced in the carpal tunnel to compare with distal radio-ulnar joint, also flattening ratio of median nerve were increased in the carpal tunnel in CTS patients. But in normal volunteers, all findings were not changed significantly. The palmar deviation and bowing ratio of transverse carpal ligament of CTS patients were about 2 times more displaced to palmar side than normal volunteers and it means increasing pressure in the carpal tunnel in CTS patients. All of these findings were most reliably judged and best visualized at the level of hook of hamate. In statistical analysis, there were significant difference between CTS patients and normal volunteers. According to results of this study, careful evaluation of MR images of suspicious patients of carpal tunnel syndrome will be one of reliable diagnostic method.
