Nonunion of the Carpal Scaphoid Fracture Treated with Herbert Screw

Abstract
The scaphoid fracture is commonest carpal bone fracture in young men, who have the highest incidence of nonunion, take the longest time to unite and spend the longeat time as outpatients. All but a few fractures are visible on the first radiograph and failure of visualization at this stage is not associated with a bad outcome. The justification for treating scaphoid fracture have been that a nonunion would most probably result in carpal osteoarthritis and pain in the wrist. We had treated 17 patients who had nonuions of the carpal scaphoid by Herbert screw fixation and autogenous iliac bone graft from March 1992 to October 1996. 17 patients were followed up average 34 months. Average union time was 9.6 weeks after operation. 13 patients were totally free of both at rest and at exercise and four patients noted moderate pain at excercise. All patient were satisfied with the results of operation and none felt worse. Treatment of scaphoid nonunion with curettage of the scar tissue and iliac bone graft and Herbert screw fixation seems to be encouraging to regain the normal anatomy of scaphoid and function of the wrist.
