Abstract |
Purpose To evaluate the effectiveness of treatment for enchondroma of the hand with curettage and dehydrated alcohol instillation. Materials and Methods: 29 patients (32 cases) with enchondroma of the hand who underwent curettage and dehydrated alcohol instillation from 1997 to 2002 were investigated. Mean follow-up period was 25 months (13~66 months). The bone healing and the degree of new bone formation were evaluated according to the criteria of Tordai. Functional results were assessed according to the four criteria described by Takigawa: appearance, active range of motion, grip and pinch, radiographic evidence of healing. Results: The number and location of the lesions were as follows: 23 in the proximal phalanx, 7 in the middle phalanx, and one distal phalanx and metacarpal. Ten lesions presented as pathologic fractures. According to the Tordai’s criteria, 23 cases (79%) were group I, and 6 –148 –cases (21%) were group II. There was no case of group III, which means recurrence. Bone healing was achieved at average 4 months after operation. Functional results were excellent in all except two which were good. There was a moderate contracture and limitation of motion of the joint of patients with multiple involvements. Average 10 degree of motion loss in the adjacent joints was found in six patients who have pathologic fractures at the time of diagnosis. During the follow-up periods, no deformity and local recurrence were seen in any patient. Conclusion: We believe that intralesional dehydrated alcohol instillation can be used as an adjuvant therapy for treatment of enchondroma of the hand without compromising hand function and complications. |
