Percutaneous Scaphoid Fixation: Dorsal Approach using Herbert Screw

Abstract
Introduction: The clinical and radiologic results of the scaphoid fractures treated by percutaneous Herbert screw through dorsal approach have been investigated retrospectively. Materials: There were nine men and one woman of an age ranging from 15 to 45 years. There were 3 acute stable, 1 acute displaced fracture and 6 delayed unions which were older than 4 weeks. A small transverse skin incision on about 1cm length is made at the dorsal scapho-lunate area. The wrist is pronated and flexed until the scaphoid is seen as a circle. The center of the circle is the target point for insertion of the guide wire into the proximal pole of the scaphoid using fluoroscopy. The screw was inserted by free hand method. Results: The average follow-up time was 15.3 months (12∼28 months). All fractures healed completely and an average time to union was 8 weeks (6∼12 weeks). Average time for return to work was 17.7 days (5∼35 days) 9 patients presented excellent results at final follow up. Conclusion: Dosal percutaneous Herbert screw fixation can provide patients faster rehabilitation, earlier return to work, and faster bone union, which is one of good treatment modalities .
